Z形成術による長さの獲得
Z形成術は2つの三角形皮弁を転位させて、瘢痕や拘縮を延長し方向を変える手技です。理論上の縦方向の長さ獲得は、中央辺に対する側方辺の角度によって決まります。
辺の角度による長さの獲得
古典的な理論上の獲得率は、30°で25%、45°で50%、60°で75%、75°で100%、90°で120%です。60度が一般的な妥協点で、角度を広げるほど長さは得られますが、閉創時の緊張が増します。
Z形成術が用いられる場面
Z形成術は線状瘢痕拘縮の解除、皮膚割線を横切る直線状瘢痕の分断、そして再建外科・美容外科における組織の再配置や延長に用いられます。
実際の長さの獲得は、皮膚の弾力性のため理論値より小さくなるのが通常です。教育目的のみ — 患者の組織と緊張度に合わせて計画してください。
